Criteria for applicants:

1) Current high school sophomore.

2) You qualify for free/reduced lunches as part of the National School Lunch Program OR you attend one of the following CMS Title-1 High schools: West Charlotte, Garinger, Vance, Harding, or West Mecklenburg.

3) Must be able to attend the 8 Saturdays on which PATCH program sessions will be held (dates listed below), and be willing to participate in additional personal and professional opportunities throughout your junior and senior year.

 

First Round Applications Due Mid-November

Step 1) Complete the PATCH Program Application online (click for link)

*Should you want to prepare your responses in advance, a PDF of the application (including short answer questions) can be found HERE. 

Step 2) Request one teacher to complete and submit a Teacher Evaluation. (This can be submitted online, and the link for teacher evaluations can be found under the  "For Teachers" tab above)

Step 3) Obtain 1 unofficial School Transcript and upload a copy to your online PATCH Program Application (we suggest that you take and upload a picture of your transcript with your cell phone or mobile device. Please ensure your cumulative GPA is cleraly visible in the transcript picture you upload.)

Second Round Interview Day December 8th

Applicants will be alerted by November if they are offered an invitation to participate in the second round Interview Day at CMC-Main Hospital. The Interview day will take place from 8am-12:30pm on December 8th, 2018, during which time applicants will be interviewed by various PATCH Coordinators, medical students, and advisory board members. Interview Day attendees will also have the opportunity to participate in some hands-on learning experiences and talk with PATCH Alumni about their experience with the PATCH Program.

1) Completed Carolinas Healthcare System Volunteer Application (Click HERE to download) and bring with you to the Interview Day at CMC-Main Hospital.

2) Proof of Flu Shot and negative TB Screening (provided for free at CMC's Employee Health Clinic)
